On Sunday, April 21, 2019 Mrs. Hendrika “Rita” Middelhoek passed away peacefully at Wainwright Long Term Care, Wainwright, Alberta at the age of 95 years.
Rita is survived by her loving family; five children, Jan Middelhoek, Henry Middelhoek, Hennie (Ernie) Boizard, Liz (Doug) Frederick, and Denise (Dale Chabaniuk) Ralph; ten grandchildren; fifteen great grandchildren as well as numerous nieces, nephews, extended family and friends.

In keeping with Rita’s wishes, no formal services will be held.
